Welcome to the 2018 online renewals for the
Alabama Board of Medical Examiners and Medical Licensure Commission


It is the responsibility of the licensee to renew all licenses and registrations. It is the stance of the Alabama Board of Medical Examiners and the Medical Licensure Commission that all renewals must be completed by the licensee.


Important Information PLEASE READ! - Renewals are different for 2018

  • The blue button below or at the bottom of the page, "Licensee Profile", will take you to the profile page for the online renewals.
  • You only need your last name and the last five (5) digits of your Social Security Number to login to the profile page.
  • Once logged into the profile you will have to verify and update addresses and answer the required demographic questions.
  • Once the addresses and demographic information has been updated on the list of licenses and registration you need to click on the renewal icon under the renewal column.
  • Each renewal will need to be processed one at a time starting with the primary license.
  • After all renewal questions have been answered and payment made the next license or registration can be renewed.
  • Postcard Reminders are the only notices you will receive. Manual forms will no longer be mailed out as final notice.

Paying by Check

Please following these steps to pay by check:

  • The licensee will log into the profile using the "Licensee Profile" button below.
  • Once logged into the profile page answer and update the profile information and questions.
  • Once the profile information has been reviewed and updated click on the icon in the "Renewal" column next to the license in the "License Information" section.
  • NOTE: The primary license has to be renewed before the renewal icon for sub licenses will appear.
  • Go through the and answer all the renewal questions and accept the review page.
  • After the review page is the payment screen. On the payment screen check the agreement message.
  • Check the box next to "I will send my payment by check" and click "Submit"
  • Read the important information and instructions on the "Alert Message" and click "Ok".
  • On the next screen click on the icon and text in red that says "Print Invoice".
  • Mail in the invoice and check to complete the renewal.
  • Invoice can be reprinted or also printed from the "Renewal History" section of the profile page. Click the printer icon in the "Invoice" column.

Print a Receipt or Copy of Renewal

Please following these steps to print a copy of your renewal or receipt:

  • Log into the "Licensee Profile" using the button below.
  • Once logged into the profile page scroll all the way to the bottom to the "Renewal History" section.
  • The renewal history section will show all licenses that have been renewed.
  • To the right of each license click on the printer icon that is under the correct column.
  • The printer icon in the "Renewal" column will print the renewal questions and responses.
  • The printer icon in the "Receipt" column will print the receipt for the renewal.

Information You Will Need to Renew

MD, DO and Limited Licenses

  • Depending on how some of the questions are answered you may need your NPI number.
  • Board Certification issue date, expiration date and document (Certification must be recognized by American Board of Medical Specialties or the American Osteopathic Association.

ACSC, QACSC Licenses

  • Your Federal DEA license number and expiration date.
  • QACSC ONLY - Course names, course dates and documentation of the four (4) AMA PRA Category 1 Credits™ regarding the prescribing of controlled substances that have been taken in the last 2 years.

CP Registrations

  • The number of hours per week you work under each CP agreement.

PA Licenses

  • The number of hours per week you work under each RA agreement.

Limited and RSV Licenses

  • Have to submit a COQ Renewal Application before these license can be renewed. Contact the Program or Facility Director for the application.

Licensee Profile




Request "Active - Non Renewal by Request" status

If you do not practice in Alabama and do not wish to renew your license you can do one of the two options:

  • Disregard all renewal notices and on February 1 (after renewal grace period) non-renewed licenses will be Inactive-Failed to Renew.
  • Request "Active-Non Renewal by Request" status and your will not receive renewal notices and your secondary licenses will also be placed on non renewal status and on February 1 (after renewal grace period) your license will be "Inactive - By Request".

Note: If at a later time you decided you want the license returned to Active status you will have to reinstate the license



Declaration of Citizenship and Lawful Presence of an Alien

Alabama Law - In order to renew a primary license (MD, DO, PA, AA, SP, RSV), the Board must have on file a valid Declaration of Citizenship and Lawful Presence of an Alien form. Visit our Declaration of Citizenship and Lawful Presence of an Alien page for more information.



Site Security

The online renewal site is secured by the use of HTTPS software and certificate. Any information entered for renewals is secure, and unavailable for retrieval by outside parties. When you click on the "Licensee Profile" link you will be taken to the secure site where you will be asked to enter your login information. On the login page you will be able to view the security certificate information in the top left hand corner of your browser window. PLEASE NOTE: The web address will not be www.alrenewals.org, this is by design.



Renewing ACSC or QACSC

In order to renew an Alabama Controlled Substances Certificate (ACSC) or a Qualified Alabama Controlled Substances Certificate (QACSC) you will have to:

  • Have a current registration to access the Alabama Prescription Drug Monitoring Program (PDMP)
    To Register:
    1. Go to: http://www.adph.org/PDMP/ and click on PDMP Login in the orange menu banner on the left
    2. Click on Practitioner /Pharmacist.
    3. Click on Registration Site for New Account.
    4. Enter username newacct and password welcome.
    5. Complete application.
      1. Choose health profession license type from the drop down box.
      2. A delegate will not enter a license number nor a DEA number.
      3. A pharmacist will not enter a DEA number.
      4. NPI is not required.
    6. If your account is approved, you will receive two emails. The first will contain your username and the second will contain a temporary password. You will be instructed to change your password when you log into the system for the first time.
    7. If you are denied access you, will be notified by email.
  • Delegate Accounts: You will not have access until your master account holder (physician) has activated and linked your account. Instructions are:
    1. Physician will log into his/her account and click on User Management the Delegate Accounts.
    2. Select the delegate account from the list on the right.
    3. Click Link Account

If you don't have these two required items you will not be allowed to complete the renewal.


During the renewal of an Alabama Controlled Substances Certificate (ACSC) and a Qualified Alabama Controlled Substances Certificate (QACSC) you will be asked for your Federal DEA Number and Expiration Date -- please have this information available.



Late Fees

  • A $100.00 late fee will go into effect at midnight (Central Time) Dec. 31, 2017, for all MD, DO and limited licenses that have not been renewed.
  • The $100.00 late fee will automatically be added when processing your renewal online.


License Expiration Information

  • MD and DO physicians
    • All licenses expire at midnight (Central Time) on Dec. 31, 2017, if they have not been renewed.
    • Medical licenses (MD and DO) have a 31 day grace period and the physician can continue practicing until Jan. 31, 2018.
    • ACSCs (Alabama Controlled Substances Certificate) DO NOT HAVE A GRACE PERIOD and expire Midnight Dec. 31, 2017. No controlled substance prescriptions are authorized after that date until your renewal is processed.
       
  • Limited license physicians
    • All limited licensees must renew their Certificates of Qualification (COQ) each year.
    • If you did not renew your COQ (and pay the COQ renewal fee) your license will expire at midnight (Central Time) on Dec. 31, 2017.
    • There is NO GRACE PERIOD, and you can no longer practice medicine until you reapply for a limited license.
    • You should contact your program coordinator for assistance in reapplying for a limited license.
    • Limited licensees that have renewed their COQs do have a 31 day grace period to renew the limited license, and the physician can continue working until Jan. 31, 2018.
    • ACSCs (Alabama Controlled Substances Certificate) DO NOT HAVE A GRACE PERIOD and expire at midnight Dec. 31, 2017. No controlled substance prescriptions are authorized after that date until your renewal is processed.
       
  • Physician Assistant (PA)
    • All licenses expire at midnight (Central Time) on Dec. 31, 2017, if they have not been renewed.
    • PA (Physician Assistant) Licenses DO NOT HAVE A GRACE PERIOD and you can not practice as a PA until the Board has processed your renewal in our office.
    • QACSCs (Qualified Alabama Controlled Substances Certificate) DO NOT HAVE A GRACE PERIOD and expire midnight Dec. 31, 2017. No controlled substance prescriptions are authorized after that date until your renewal is processed.
       
  • Anesthesiologist Assistant (AA)
    • All licenses expire at midnight (Central Time) on Dec. 31, 2017, if they have not been renewed.
    • AA (Anesthesiologist Assistant) licenses DO NOT HAVE A GRACE PERIOD and you can not practice as a AA until the Board has processed your renewal in our office.
       
  • ACSC or QACSC
    • All licenses expire at midnight (Central Time) on Dec. 31, 2017, if they have not been renewed.
    • ACSCs (Alabama Controlled Substances Certificate) and QACSCs (Qualified Alabama Controlled Substances Certificate) DO NOT HAVE A GRACE PERIOD and expire at midnight Dec. 31, 2017. No controlled substance prescriptions are authorized after that date until your renewal is processed.
       
  • CP agreements
    • All CP (Collaborative Practice) licenses expire at midnight (Central Time) on Dec. 31, 2017, if they have not been renewed.
    • These registrations DO NOT HAVE A GRACE PERIOD and the CP is not valid until the Board has processed your renewal in our office.
    • If you are prompted to renew a CP registration and you do not think you have a CP, contact our office–if our records indicate a CP and it is not renewed or formally terminated by Dec. 31, it will be reported to the Board (online termination form available here)
       
  • SP or RSV physicians
    • All SP (Special Purpose) and RSV (Retired Senior Volunteer) licenses expire at midnight (Central Time) on Dec. 31, 2017, if they have not been renewed.
    • All RSV licensees must renew their Certificates of Qualification (COQ) each year.
    • If you did not renew your COQ (and pay the COQ renwal fee) your license will expire at midnight (Central Time) on Dec. 31, 2017.
    • There is NO GRACE PERIOD, and you can no longer practice medicine until you reapply for a RSV license.
    • SP and RSV that have renewed their COQs have a 31 day grace period and the physician can continue practicing until Jan. 31, 2018.

You can check the status of any license by going to the Licensee Search section of the Board and Commissions main website.


Troubleshooting Information

  • Make sure your browser is current
    • In order to use this renewal site to renew, view and print forms, you must have the most current version of your browser.
    • Get the most resent version of your browser (all are free):
    • Popup blockers
      • You must allow popups or add the renewal site to your trusted sites in order to perform some of the tasks during the online renewals.
      • For instructions on how to disable your popup blocker do an internet search on disabling popup blocker for the name of browser you are using
         
    • Login problems
      • Make sure you are entering Last Name Only in the Last Name field.
      • Make sure to enter the Last Name as it was reported to the Board on the intial application.
      • Make sure you are entering Only the Last five (5) Digits of the SSN.
         
    • Error messages or other problems
      • If you are getting an error message please email the error message to .
